The File of Life The File of Life
provides emergency personnel easy access to potentially lifesaving
information in the event of an emergency. Vital information such as Name,
Religion, Medications/Dosages, Doctor's Name, Hospital, Emergency Contact
Number,
Insurance Information, Social Security Number, Medical Conditions,
Allergies, and Recent Surgeries is noted on the card and placed in a
magnetic folder outside your refrigerator. The information should be filled
out in pencil so changes can be made easily. This important data will
provide medics and other emergency personnel a quick medical history when
the patient or care giver is unable to offer one. To
register for this important program, please call
Wendi Howard at the
Coalinga Senior Center (935-5001). |

The Coalinga-Huron Recreation and Park District would like
